Hawaii's LLC reinstatement rule is in Haw. Rev. Stat. § 428-811, which allows a domestic LLC administratively terminated under § 428-810 to apply for reinstatement within two years after the effective date of termination. Form X-4 requires all delinquent annual reports, all delinquent fees and penalties, and a tax clearance certificate. The reinstatement application fee is $25 and the annual report fee is $15.
